A mental health practitioner (MHP) in a Primary Care Network (PCN) is a healthcare professional who provides mental health support within GP practices, acting as a bridge between primary care and specialist services. They assess and support patients with complex or severe mental health needs, offer advice to GPs, and help manage referrals to other services. Patients can be referred to an MHP by their GP or, in some cases, can self-refer.
